What "citation-grounded" actually requires.

Most enterprise AI tools generate answers and then optionally show "sources." The sources are often plausible-looking but unverifiable — the model picked them as relevant context, but the answer text isn't directly traceable to a specific span in a specific document.

Citation-grounded AI is structurally different.
